There are varied prices according to your document type. For example, per PDF page, you need to pay $8 which will then be multiplied to the number of pages in your document that you want to make accessible.

Yes, Files Accessibility makes PDF forms accessible as well. For your PDF form or if any page in your document is in the PDF form, you will be charged as per the rates of the PDF form, i.e., $40.
